You or any household member had any of the following symptoms of COVID-19 within the past 14 days: fever, shortness of breath, dry cough, runny nose, sore throat, body aches, diarrhea and/or vomiting, loss of sense of taste/smell
You or any household member traveled outside the United States or domestically within the United States by commercial airline, bus, ship, or train within the past 14 days
You or any household member have had contact with a person who has tested positive for COVID-19 or who is undergoing a test for COVID-19
